  • Certificates of Employment: Attach certificates from your previous employment.
  • Clearances and IDs: Include a valid NBI/Police Clearance and a copy of your Civil Service Eligibility and/or Valid PRC ID.
  • Performance Rating: If applicable, provide your performance rating from your last job.
